Does Every Thyroid Nodule Require a Biopsy?

Ultrasound evaluation of a thyroid nodule

No. A significant proportion of thyroid nodules do not require a biopsy and are simply monitored with ultrasound follow-up.

What determines the biopsy decision?

The biopsy decision is not based on size alone. The following are evaluated together:

  • The nodule's solid/cystic composition
  • Margin characteristics
  • Presence of calcifications
  • Shape and orientation
  • Risk classification (the approach recommended by current guidelines)

What does a monitored nodule mean?

Follow-up means measuring the nodule with ultrasound at set intervals and checking whether its features change. Many nodules show no meaningful change for years.

When is a biopsy more strongly considered?

  • Nodules with suspicious ultrasound findings
  • Higher-risk nodules that exceed a certain size threshold
  • Nodules showing clinically significant rapid change

Summary

  • Not every nodule requires a biopsy
  • Size alone is not sufficient
  • Ultrasound findings and guidelines are evaluated together
These assessments are based on an ultrasound examination performed by a radiology specialist. In our clinic, detailed ultrasound evaluations are carried out using an advanced ultrasound system and SMI technology. When needed, a second-opinion ultrasound approach is also offered.
